Licensed Social Worker

The Licensed Social Worker is responsible for providing direct social work services for Long Term Home Health Care Program (LTHHCP) clients and their families. You will ensure that the social service needs of clients are met.
Requirements include current registration to practice as a Licensed Social Worker in New York State and a Master’s degree in Social Work after successfully completing a prescribed course of study at a graduate school of Social Work accredited by the Council on Social Work Education and the Education Department. Additional requirements include a minimum of 1 year social work experience in a health care setting. Bilingual skills and a valid driver’s license may be required, as determined by operational/regional needs.
