Graham Windham, an award winning agency and one of NY’s most dynamic non profit organizations serving Children and Families seeks a qualified professional for our Preventive Service Program in Manhattan.
Reporting directly to the Program Supervisor or Program Director, you will provide social work intervention for client families in order to: reduce risk, promote well-being and child safety and prevent the placement of children in foster care. Responsible for the coordination of services, counseling, advocacy, recording maintenance of case records. Will assist in the administrative oversight of the program which includes providing coverage, attending meetings and community outreach. Makes comprehensive family assessments and works towards family reintegration or restoration of functioning or maintenance. Displays good knowledge of cultural, socio-economic, and psychosocial aspects of family life. Responsible for assisting program director and supervisor to carry out agency and program goals and objectives, which include submission of all required reports, FASPs and progress notes with the use of Connections (a Computerized Case Management System) and PROMIS.
M.S.W. .preferred or minimum 2 years of related experience with a B.A. degree.
Physical and sensory Requirements:
Ability to read, write and converse in English (Bilingual a plus)
Ability to evaluate/interpret information and make independent judgements/decision
Ability to deal effectively during difficult or stressful situation.
Ability to respond to telephone and other auditory stimulation's.
Ability to communicate with staff, residents, families, outside agencies and other community Programs.
